  • Services are planned and agreed upon based on individual needs
  • Services range from weekly visits to connect, manage ​medication and ensure home safety- to blocked time for clients ​transitioning to end of life.
  • Services for clients facing a terminal diagnosis are focused on ​training, family support, legacy work and assistance in ​providing comfort care.
  • Monthly enrollment in continued services or services on an as ​needed basis for respite.
  • Vigil, legacy or caregiver services at end of life are provided for ​an hourly fee.
